Balance exercises are worth doing, and the research behind them is solid. But most falls do not happen while you are concentrating on your balance. They happen while you are walking and thinking about something else. This article explains why researchers have been adding a thinking task to walking practice, an approach called dual-task training, and what the evidence says it adds.
Balance training earns its reputation
Let’s start with what standard exercise gets right. A 2019 Cochrane review, one of the most trusted forms of medical evidence, looked at 108 trials with more than 23,000 older adults. It found that exercise programs, especially those built around balance and functional movements, reduced the rate of falls by roughly 23 percent compared with not exercising. If you already do tai chi, strength work, or balance classes, that habit is paying you back. Nothing in this article suggests stopping.
But falls rarely wait for your full attention
Think about the last time you stumbled. You probably were not standing on one leg with your eyes closed. You were more likely carrying groceries, mid conversation, or working through a worry while crossing a street. Walking feels automatic, but for the brain it is real work, and that work competes with everything else you are thinking about.
Researchers call this cognitive-motor interference. When attention has to split between moving and thinking, the walking is often what gives. There is even a well known clinical observation called “stops walking when talking,” where an older adult pauses mid stride to answer a question. That pause is a measurable predictor of fall risk.
Standard balance training strengthens the body in calm, focused conditions. It does not practice the exact skill most falls test: keeping your walking stable while your mind is busy. That skill can be trained, but you have to train it directly.
What happens when you train both at once
Dual-task training does exactly that. You walk while your mind works on something, such as recalling numbers, spotting patterns, or holding a short list in memory. Over weeks of practice, the brain gets better at running both jobs at the same time.
The first thing that improves is balance itself. A 2022 systematic review in the International Journal of Environmental Research and Public Health examined 30 randomized and pilot randomized trials of dual-task training in older adults. Its headline finding was that 23 of the 30 studies, which is 76.6 percent, reported improved balance after training. A smaller number of those studies went a step further and recorded fewer actual falls, which is what you would hope to see follow from better balance under distraction.

The cleanest head-to-head test
One trial deserves special attention because it isolated the thinking layer completely. In the V-TIME study, published in The Lancet in 2016, researchers assigned 302 older adults at high risk of falling to one of two programs. Both groups walked on a treadmill for the same number of sessions. One group just walked. The other group walked while responding to cognitive challenges such as obstacles, distractions, and route choices projected in front of them.
The physical exercise was identical. Only the mental layer differed. Six months later, the group that trained with the cognitive challenges had a 42 percent lower fall rate than the group that only walked.

The evidence has kept growing
Since then the research base has widened considerably. A 2025 meta-analysis in European Geriatric Medicine pooled 44 studies covering 2,782 older adults. It concluded that dual-task exercise significantly improved balance and functional mobility and reduced how often participants fell, compared with single-task exercise or no exercise. What this means for your routine
Keep the strength and balance work you already do. It has decades of evidence behind it. The research above simply points to a gap worth filling: some of your walking time can do double duty, training your mind and your body to work together the way real life demands.
